Report a side effect.

If something does not feel right after starting a treatment, or there is a problem with a medication you received, tell us. Here is how.

Emergency

In an emergency, call 911.

If you have a severe reaction or think you may be having a medical emergency, call 911 or go to the nearest emergency room now. Do not wait for a reply to a message.

Call 911
Tell your care team

For a side effect or a problem with a medication

Tell us as soon as you notice it rather than waiting for your next check-in. Say which medication you take, what you noticed and when it started. If a medication arrives damaged, or you think something is wrong with it, contact us before you use it.

The FDA

You can also report to MedWatch

MedWatch is the FDA’s program for reporting serious problems with medical products, including medications. You can report on your own, without your provider, online or by phone. A MedWatch report goes to the FDA, not to your care team, so tell us as well.

Report to MedWatch online

Or call MedWatch at 1-800-FDA-1088 (1-800-332-1088).
